Requirements for first time applicants 


  1. Birth certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC( and photocopy of BC issued by the Local Civil Registrar (LCR) duly authenticated by NSO and any supporting document indicating full name, date and place of birth, and citizenship. 
  2. Supporting documents are the following: 
  • Voter’s affidavit or CTC of voter’s affidavit from the COMELEC; 
  • Driver’s license;
  • National Bureau of Investigation (NBI) clearance (at least 2 years old); Old SSS E-1 form;
  • Baptismal certificate; Income Tax Return (ITR); School form 137 or Transcript of Record;
  • GSIS policy; Government Service Record: Seaman’s book;
  • Marriage contract; 
  • Professional Regulations Commission (PRC) ID; 
  • Postal ID; 
  1.  Three (3) copies of the applicant’s new photo (colored with plain background) in 4.5cm x 3.5cm. Photo should show the applicant in decent attire with collar. 
  2.  Personal appearance before the passport-issuing office by the applicant except when applicant is: 
  • Not more than eight (8) years old; or, 
  • At least 65 years old; or, 
  • Mentally or physically incapacitated. 

 IN CASE OF NO BIRTH RECORD 
For applicants born on or before the year 1950:
  •  A certificate of non-availability of birth record from the NSO may be submitted. 
  • Joint birth affidavit signed and executed by two (2) disinterested persons. 
  • Baptismal or CTC of voter’s affidavit from the COMELEC or any public document indicating date and place of birth and citizenship. 

 For applicants born after 1950:
 In case of birth certificates with delayed registration, submit authenticated birth certificate and supporting document indicating date and place of birth and citizenship. 
  1. Additional requirements 
  • For married women: Submit marriage contract (MC) in security paper issued by the NSO or CTC issued by the LCR duly authenticated by the NSO. 
  • For married women opting to use maiden name: 
  • Submit MC and affidavit indicating that she has not used her married name. 
  • For women who obtained annulment or are divorced from their foreign husbands: Submit annotated MC in security paper (SECPA) and: 
  •  Annulled Authenticated copy and photocopy of the first page and the dispositive portion of the judgment granting divorce or annulment; 
  •  Divorced CTC and photocopy of the first page and the dispositive portion of the judgment granting divorce or annulment, authenticated by the Philippine Embassy/Consulate where the divorce was obtained. 

  1. For fiancĂ©es and spouses of foreign nationals: Submit original and photocopy of Commission on Filipinos Overseas (CFO) Guidance and Counseling Certificate of Attendance. 
  2. For minors (below 18 years old): 
  • Personal appearance of either parent; 
  • If minor is not traveling with either parent, submit an original and photocopy of DSWD clearance and affidavit of support and consent indicating the name of the traveling companion and relationship to the minor. 
  • If both parents are abroad, then an affidavit must be authenticated by the nearest Philippine Embassy or Consulate General. 
  • If both parents are abroad, then the person applying in behalf of the minor must submit a Special Power of Attorney authenticated by the nearest Philippine Embassy or Consulate General. Photocopy and original passport of the person traveling with the minor. 
  • If minor is eight (8) years old and below and traveling unaccompanied by the mother, the mother’s consent is required. 
  • For legally adopted children: Authenticated copy and photocopy of the Court Decision/Order on Adoption and Certificate of Finality; NSO-issued or Authenticated New Birth Certificate; DSWD permit/clearance for minor children if traveling with parent/persons other than parent/s.  
  • For illegitimate minor children: Personal appearance and written consent of the mother; DSWD clearance if minor is traveling with the father/other persons. 
  • For legitimate children due to subsequent marriage of the parents: Authenticated birth certificate with annotation of the amended surname of the child; Marriage contract of the parents. 
  1. For Muslim applicants: 
  • For those whose births were registered, follow the requirements for new applicants.
  • For those whose births were not registered: CTC of late registered birth certificated from the NSO; Original and photocopy of voter’s affidavit or NBI; Certificate from the Office of Muslim Affairs for Muslim Converts.
  1. For converts: 
  • Submit annotated BC in SECPA; 
  • Court Order to change name; 
  • Shari’ah Court Order. Additional supporting documents may be required when necessary. 
  1. Requirements for renewal of passport: 
  • Personal appearance of the applicant shall not be required. The application may be filled by: 
  1. Any agency duly accredited with the Department of Foreign Affairs; or, 
  2. Member of immediate family of the applicant authorized in writing by the latter (applicant’s spouse/brother/sister/parent/children/legal guardian/guardian having custody of the child); or, 
  3. A person authorized in writing by the applicant provided the release of passport is only to applicant. 
  •  Personal appearance may be required as warranted by circumstances. 
  1. Three (3) copies of 4.5cm x 3.5cm new photos (colored with plain, white background). Photo should show applicant in decent attire with collar. 
  2. If passport being renewed is brown or issued prior to 01 May 1995: Present old passport and photocopy of pages 1, 2, and 3 (amendment). The pages showing the latest Bureau of Immigration departure and arrival stamps. (Shall not be required if applicant is applying in person) Supporting document with complete middle name. 
  3. In case passport being renewed is green or issued after 01 May 1995: Present passport and photocopy of pages 1, 2, and 3 (amendment) inside and back covers. The pages showing the latest Bureau of Immigration departure arrival stamps. (Shall not be required if applicant is applying in person).
  4. For married women who chose to adopt surname of husband in her new passport: 
  • Marriage contract (MC) in Security Paper (SECPA) issued by the NSO; or, 
  • CTC and photocopy of NC issued by the NSO. 
  1. For minors (below 18 years old): 
  • Personal appearance of either parent; 
  • If minor is not traveling with either parent, submit an original and photocopy of DSWD clearance and affidavit of support and consent. If both parents are abroad, then such affidavit must be authenticated by the nearest Philippine Embassy or Consulate General. 
  • If both parents are abroad, then the person applying in behalf o the minor must submit a Special Power of Attorney authenticated by the nearest Philippine Embassy or Consulate General. 
  • Passport and photocopy of the passport of the person traveling with the minor. 
  1.  Requirements for replacement of lost passport 
  • If already expired, then submit a notarized affidavit of loss. No penalty fee of Php 200.00. 
  • If still valid, submit a police report and a notarized affidavit of loss. There will be a 15-day clearing period prior to the processing of application for a new passport. 
  • Personal appearance. 
  • BC in SECPA and supporting documents. 
  • Three (3) copies of 4.5cm x 3.5cm new photos (colored with plain, white background). Photo should show applicant in decent attire with collar. 

Travel Tips and Guide 


  1. CHECKLIST OVERVIEW 
  • IF TRAVELING INTERNATIONALLY 
    • Verify if your passport and visa requirements are in order. 
    • Visit your doctor for any necessary inoculations and obtain medicine and a medical certificate if needed. Find out departure tax details for all countries you will be visiting. 
  1. PRIOR TO DEPARTURE 
    • Order special meals, request a seat assignment and any special assistance e.g. wheelchair, etc. if required. Provide relatives or friends with a detailed copy of your itinerary including flight details, accommodation addresses and telephone numbers. 
    • Your ticket contains vital information about your flight. It is a good idea to keep a separate note of your ticket number with you. Your boarding pass for your flight will be issued when you hand over your ticket at the airport check-in counter. Your baggage receipts will be attached to your ticket and it will be handed back to you. 
    • Arrange travel assistance. 
    • Ensure your credit cards are valid for the duration of the trip and purchase traveler's cheque if required. 
  1. ON THE DAY OF THE TRAVEL 
    • Check again that you are carrying all travel documents, tickets, passport, money and medications in a safe place. 
    • Arrive at the airport at least 3 hours before departure. 
  1. AT THE AIRPORT 
    • Put a lock on your luggage as you may get something stolen or someone may smuggle contraband in your luggage. 
    • Never lose sight of your luggage, even at security checks. 
    • If you have requested special assistance, identify yourself to the agent at the check-in counter. 
    • Pay departure tax where necessary before proceeding to immigration. Complete departure card, if applicable. 
  1. PACKING GUIDE 
    • CLOTHES 
      • Bring clothing that best suits the climate condition of the place you're going. Make it a habit to check first the weather daily report through the Internet or from your local TV or radio news group. This would help you prepare the necessary wardrobe that you might want to bring for your trip. 
    • ELECTRICAL APPLIANCES 
      • If you attend to pack items as hairdryers or electric shavers, remember that the countries you are visiting may use different voltages and outlet types. 
      • Adaptors are usually available at shops selling travel goods, at duty-free stores or at airport shops. 
    • BAGGAGE 
      • All your baggage, whether checked in or with you in the cabin, should be labeled internally and externally with your name and address. There are certain articles that must not be carried in any of your baggage. You will find details on such "Dangerous Goods" in your ticket folder. 
      • Never carry luggage for other people and make sure that you pack your own bags. Unwary travelers have duped into carrying and checking in bags and parcels containing drugs or prohibited items. It is wise to lock your own baggage. 
      • Information on restricted imports and exports can be obtained from consulate of countries you intend to visit. 
      • Your ticket shows the baggage allowance according to your transportation class and your destination. 
  •  Here are some tips for you to follow: 
    • Prefer hard-shelled baggage and always make sure they are locked. 
    • Put a name and address tag inside and outside your baggage. Carefully keep the baggage tags given to you at the check-in desk. 
    • Fragile objects such as crockery, electronic material, bottles and paintings must NOT be put into your checked baggage. 
    • Heavy items (over 32k) or bulky items (surfboard, bikes, golf bags) are to be declared when making a reservation. 
    • The following objects are to be carried in the hold with the airline's agreement – knives, scissors, firearms, cartridges. 
    • These objects are forbidden in the hold and in the cabin- explosives, oxidizers, corrodents, fireworks, toxic or infectious substances, compressed gases, poison, flammable liquids and solids and radioactive material, toiletries (aerosols, toothpaste, perfumes), thermometer for medical use, lighter and matches. 
  •  Check-in Baggage 
    •  It depends on the class of service, airlines and destination. Please contact us for the accurate baggage allowance. 
  •  Carry-on Baggage 
    •  Each passenger is allowed to carry one piece of carry-on luggage small enough to be placed in the overhead rack or under the passenger seat of the aircraft cabin. The carry-on luggage must be within the following specifications: 
      •  size (length+width+height) shall not exceed 45 in. or 115 cm. 
      • weighing not more than 7 kgs. or 14 lbs. 
  •  If carry-on luggage fails to comply with the required applicable dimensions and weight, carry-on luggage will be checked in and charged with the corresponding excess baggage charges (if applicable). In addition to the free carry-on luggage allowance, a passenger is allowed to carry the following items onboard: 
    • laptop with case 
    • a small handbag 
    • a coat 
    • wrap or blanket 
    • a walking stick or a pair of crutches 
    • a small camera or a pair of binoculars 
    • a reasonable amount of reading materials 
    • infant's food and carrying basket
  •  The following necessities are to be carried in the cabin: 
    • Valuables - cash, currencies, jewels, laptop. 
    • In flight necessities - baby-food and nappies, medicine, books or magazines. 
    • Indisposable objects – passport or identity papers, home or car keys. 
  •  Lost Luggage 
    •  The time limit for reporting lost, damaged, delayed or missing luggage within 24hours of occurrence. The preliminary notification must be followed by a written notification within 45 days for domestic travel and 21 days for international travel. 
    •  Liability for loss, delay, or damage to baggage is limited unless a higher valuation is declared in advance and additional charges are paid. For most international travel (including domestic portions for international journeys) the liability limit is approximately US$ 9.07 per pound for checked baggage and US$ 400 per passenger for unchecked baggage. 
    •  For travel wholly between U.S. points, federal rules require any limit on an airline's baggage liability to be at least US$ 2,500 per passenger. Excess valuation may be declared on certain types of articles. Some carriers assume no liability for fragile, valuable or perishable articles.
